How to Leave Feedback

Go to http://www.swaylocks.com/cgi-bin/surfshop/feedback.cgi.

You'll be prompted to log into your account.

After logging in, you'll see a list of all of the orders that need feedback.

Find the order on the list and click the "Leave feedback" link on the right.
